Program Description

The Hypertension Program provides education; self-management and skills training, providing a framework for the screening, early diagnosis, treatment and ongoing management for patients with Hypertension. The process involves recording a detailed health history, risk factor assessment (using Framingham and/or SCORE Canada risk assessment tool), target organ damage assessment, screening for signs of secondary hypertension, physical assessment and ordering routine laboratory tests. Staff follow the CHEP (Canadian Hypertension Education Program) guidelines. Patients are supported in maintaining lifestyle changes over the long term.

To ensure early identification and treatment, reduce morbidity and improve quality of life.

SMART Objectives

• Increase access by early identification through screening and assessment

• Reduce BP: < 130/80 for diabetics; <140/90 for non-diabetics; and < 150/90 for patients > 80yrs

• Increase patients options re healthy lifestyle habits

• Decrease the progression of atherosclerosis, CAD, dementia and other related conditions; minimize need for medications and more intrusive interventions
