MEMORY ASSESSMENT SERVICE

Current best practice guidelines for comprehensive dementia care recommend the timely combination of pharmacological interventions and psychosocial measures such as counseling and social supports. Desirable outcomes of dementia treatment are twofold: enable patients to live independently at home for as long as possible and reduce the challenges for care providers. Primary care detects and diagnoses dementia; provides information to patient and family members; implements and monitors pharmacological treatment and organizes linkages for further counseling and/or supportive services. A systematic, interdisciplinary collaboration between primary care, medical specialists and relevant professionals in the field of dementia care is integral for optimal care provision. The Memory Assessment Service at PFHT enhances access; provides early identification by determining whether the patient is suffering from a memory disorder; organizes appropriate treatments and follow-up and systematically monitors and adjusts the care plan, aligning with the patient’s and family’s changing needs.
