Preventative Health Care
“Your Health Is Our Concern.” You may be contacted as a reminder to complete certain procedures for preventative care. The Ministry of Health and Long Term Care recommends certain tests and procedures be done on a regular basis to prevent serious illnesses. These include occult blood, colonoscopies, pap smears, mammograms, bone densities, immunizations for babies and children, flu and COVID-19 vaccinations. The AstraZeneca vaccine is now available for eligible patients. See the Brockville clinic staff provide a convenient drive through service for your protection!
Preparing For Your Appointment
Please update your contact information at Reception upon arrival - please provide your email (or an email we can use to contact you) as this is the most effective way to communicate (especially during this pandemic). Please bring all your medication whether prescribed, over the counter, supplements, etc., to all appointments. It will help your Physician or Healthcare Professional, if you are prepared and have organized your questions prior to your office visit.
Health Cards
Please have your Health Card ready for all appointments and ensure it is valid prior to your arrival. If your Health Card is not valid, contact the Ministry of Health and Long Term Care at 1-800-268-1154 or contact the nearest Service Ontario office (Prescott Service Ontario, telephone number: 613-925-3341) to renew your Health Card.
Prescription Renewals
For prescription renewals, please contact your Pharmacy and the Pharmacy will fax the request to our office. Contacting the Pharmacy a few days before the prescription runs out ensures that you do not run out of medication.
Narcotic Medication
Narcotic medication will be monitored for your protection. By law, narcotics cannot be prescribed over the telephone. Only limited quantities will be prescribed at one time. Renewals are made by contacting the Pharmacy. Patients are required to sign a Narcotics Agreement and random blood work will be ordered.
Referrals To Specialists
Our team members make referrals for patients to specialists as needed. You will be notified by the specialist of your appointment. Please do not call the office. If you are unable to attend the appointed date and time, it is your responsibility to contact the specialist's office to cancel or rebook your appointment.
Laboratory and X-ray Results
Our office will contact you to follow up after tests, if this is required.
Post Hospitalization Follow Up
You are encouraged to call Reception and book a follow up appointment within 7 days of discharge to ensure continuity of care. Our Nurses may call you as a reminder.
