MEDICAL OFFICE ASSISTANT

POSITION SUMMARY

The Medical Office Assistant (MOA) provides front-line administrative and patient-focused support to the Prescott Family Health Team. Working collaboratively with the Administrative Team, the MOA contributes to the smooth and efficient day-to-day operations of the clinic.

Responsibilities include appointment scheduling, patient registration, medical records, referrals, correspondence, documentation, and general administrative support. The MOA will also provide administrative coverage and assist with workload redistribution as needed to support the overall functioning of the clinic.

The position works closely with administrative staff, physicians, Nurse Practitioners, nurses, and other healthcare professionals to ensure patients receive timely, respectful, and professional service.

KEY RESPONSIBILITIES

Patient & Front-Line Support

  • Greet and assist patients in a professional, welcoming, and respectful manner.

  • Answer and direct telephone calls, respond to inquiries, and provide information as appropriate.

  • Provide patients with information regarding appointments, services, and clinic processes.

  • Support a positive and patient-centred experience throughout the patient's interaction with the clinic.

Scheduling & Administrative Support

  • Schedule and manage patient appointments, including follow-ups and referrals.

  • Register patients and maintain accurate demographic and contact information.

  • Process referrals, consultations, reports, and other clinical documentation.

  • Support physicians and healthcare providers with administrative and clerical requirements.

  • Assist with forms, documentation, scanning, filing, and data entry.

  • Manage incoming and outgoing correspondence, faxes, and electronic communications.

  • Coordinate communication between patients, providers, specialists, pharmacies, and community partners.

  • Assist with patient recalls, reminders, and follow-up activities.

  • Provide administrative coverage and support workload redistribution within the Administrative Team as required.

Medical Records & Documentation

  • Maintain electronic medical records and ensure information is entered accurately and efficiently.

  • Ensure clinical and administrative documentation is processed and maintained appropriately.

  • Follow organizational procedures related to privacy, confidentiality, records management, and information security.

Team & Organizational Support

  • Work collaboratively with administrative staff and the interprofessional healthcare team.

  • Participate in team activities and initiatives as appropriate.

  • Maintain confidentiality and comply with all applicable privacy, health and safety, and organizational policies.

  • Perform other administrative duties and projects as assigned by the Administrative Lead/Manager or Executive Director.

PROGRAM ASSISTANT

POSITION SUMMARY

The Program Assistant provides administrative and patient-focused support to the Prescott Family Health Team, assisting with the coordination and delivery of programs, services, and initiatives.

The position works closely with healthcare providers, staff, patients, and community partners to help ensure programs operate efficiently and provide a positive experience for patients and participants.

The successful candidate will be organized, adaptable, and comfortable working with patients, staff, community partners, and healthcare professionals in a dynamic primary care environment.

KEY RESPONSIBILITIES

Program & Patient Support

  • Coordinate and support patient programs, workshops, education sessions, and group activities.

  • Assist patients with program inquiries, registrations, appointments, and follow-ups.

  • Provide information regarding PFHT programs, services, and relevant community resources.

  • Support patient communication and engagement related to programs and services.

  • Help create a welcoming, respectful, and patient-centred experience for program participants.

Program Administration & Documentation

  • Maintain program records, attendance information, data, and reporting requirements.

POSITION SUMMARY

The Nurse Practitioner will play a key role in providing comprehensive, patient-centred primary care to individuals and families in the Prescott community.

The successful candidate will practise autonomously and collaboratively within their professional scope, providing assessment, diagnosis, treatment, health promotion, disease prevention, and ongoing management of acute and chronic conditions.

As the Most Responsible Provider (MRP), the NP will establish and maintain their own roster of patients and will be responsible for coordinating and overseeing their patients' ongoing primary care needs.

This position offers an opportunity to work within a supportive interprofessional team while making a meaningful contribution to improving access to primary care in the community.

KEY RESPONSIBILITIES

Clinical Care

  • Provide comprehensive primary care services to rostered patients, including health history, physical assessment, diagnosis, treatment, and follow-up.

  • Assess, diagnose, and manage acute and chronic health conditions using evidence-informed clinical decision-making.

  • Order, interpret, and act upon appropriate diagnostic investigations.

  • Prescribe medications and initiate, modify, or discontinue treatment within the NP's legislated scope of practice and individual competence.

  • Provide health promotion, disease prevention, screening, counselling, and patient education.

  • Provide timely assessment and management of urgent and episodic health concerns as required.

  • Recognize and respond appropriately to conditions requiring consultation, referral, or care beyond the NP's scope of practice.

Patient Care Coordination & Collaboration

  • Develop and implement individualized, patient-centred care plans in partnership with patients, families, caregivers, and the interprofessional team.

  • Coordinate care and promote continuity across primary care, specialty services, hospital services, and community resources.

  • Communicate significant clinical findings, treatment plans, and follow-up requirements effectively with appropriate members of the care team.

  • Facilitate referrals, consultations, transitions in care, and appropriate follow-up.

  • Maintain accurate, timely, and comprehensive clinical documentation in accordance with CNO standards and PFHT policies.

  • Current Basic Cardiac Life Support (BCLS) certification, required.

  • Professional liability protection appropriate to NP practice, required and maintained throughout employment.

  • Membership with the Nurse Practitioners' Association of Ontario (NPAO) considered an asset.

ADDITIONAL REQUIREMENTS

  • Minimum 3–5 years of recent clinical experience, with primary care experience strongly preferred.

  • Valid driver's licence and access to reliable transportation.

POSITION SUMMARY

The successful candidate will provide professional social work services in accordance with the standards of practice of the applicable regulatory college. Working as part of an interprofessional team, the clinician will provide assessment, therapeutic intervention, education, health promotion, case management, and system navigation to patients and families experiencing mental health concerns.

The successful candidate will work with individuals and groups and will contribute to the development, implementation, and evaluation of the Family Health Team's mental health programming.

KEY RESPONSIBILITIES

Clinical Care & Therapy

  • Provide evidence-based cognitive behavioural therapy (CBT) and other appropriate therapeutic interventions in individual and group settings.

  • Provide assessment and treatment for patients experiencing a range of mental health concerns.

  • Conduct comprehensive psychosocial assessments of patients with known or suspected psychiatric and/or mental health conditions.

  • Establish and maintain therapeutic relationships with patients and, where appropriate, their families and caregivers.

  • Develop individualized treatment plans in collaboration with patients and the interprofessional team.

  • Provide clinical interventions designed to promote recovery, resilience, community integration, and improved functioning.

  • Apply appropriate therapeutic approaches based on patient needs, clinical presentation, and professional scope of practice.

Care Coordination, System Navigation & Patient Support

  • Provide resources, case management, and system navigation support as required.

  • Assist patients with completion of ODSP applications, medical forms, disability-related documentation, and other insurance or benefit forms, as appropriate within the clinician's professional scope.

  • Support patients in understanding and navigating available disability, income-support, insurance, and community benefit programs.

  • Liaise with community agencies, healthcare providers, hospitals, insurance providers, and other partners to support continuity of care.

  • Contribute to case formulation and development of integrated plans of care with the interprofessional team.

  • Experience with dialectical behaviour therapy (DBT) or other evidence-based therapeutic approaches is considered an asset.
