Overview: This is an excellent opportunity to work as a physician in a beautiful setting, along the expansive shores of the St Lawrence River. The Prescott Family Health Team (PFHT) clinic is situated in Southeastern Ontario, on St. Lawrence River parkland, adjacent to a beach, swimming pool, boat launch and tennis courts. Prescott is strategically located one hour east of Kingston, 45 minutes south of Ottawa, five minutes from the United States Border and is at the intersection of two major highways: 401 and 416. Our location offers small town living and easy access to big city amenities. PFHT is composed of a dynamic group of health care providers focused on delivering primary health care through the collaborative efforts of 4 family physicians and interdisciplinary health care providers, supported by our hospital and community partners and managed by an Executive Director. As a member of the Prescott Family Medical Associates, (PFMA) and Prescott Family Health Team Board of Directors, you will deliver primary health care services including preventative care programs, chronic disease management and acute and episodic care. This is the ideal clinic for a physician with a long-term goal and new graduates are welcomed.


Duties and Responsibilities: You will provide clinical leadership, work in conjunction with a team of interdisciplinary health care providers, navigating and coordinating patient’s care through the healthcare system. As a physician you will provide a comprehensive approach to the management of prevention, illness and chronic disease and oversee patient care with both simple and complex multi-system problems. You will conduct health assessments, perform physical exams, electronically documenting a full history including presenting complaints; provide continuous care including diagnosis and treatment; and offer ongoing management for a range of medical problems from minor ambulatory care visits to severe life threatening illness. Services include acute medical care; primary reproductive care; palliative care; support for hospitals, home and long term care homes; service coordination and referral; patient education and preventative care; prenatal, obstetrical, post-natal care. You will ensure that patients have access to all the programs and services offered by the PFHT of interdisciplinary health professionals.


Qualifications and Requirements: The successful candidate must be licensed or eligible for license to practice in Ontario with accreditation in Family Medicine by The College of Physicians of Canada (CCFP). Excellent interpersonal skills and a commitment to inter-professional care are essential.

Salary Range: A Blended Capitation model is available to compensate eligible physicians who practice full-time; you will be a member PFMA and responsible for enrolling your own patients.
